what they mean when they say "the left is afraid of free speech" is really that milo and his nazis are afraid of the consequences of free speech.

they are free to spout their nazi gibberish, they just have to accept that it is unprotected hate speech and might earn them a beating.

Also, I am not sure that picture matches up. Why would someone take 2 pictures of his head and then body separately? It looks like a selfie pic, so why can't we see the full picture in the bottom one with his head? Whoever "leaked" this definitely would have the full picture.
