I'm throwing together a website for medical marijuana patients. Essentially you go to the website pick your state, then pick which form you need (be it new patient application, renewal, denial appeals, etc).
The patient will then be shown a web form, that mirrors the form they're requesting. After filling it out they will be given a PDF of the form, with all the information they filled in, ready to print and sign.
Does anyone have any hosting they can donate? I just bought mmforms.info (couldn't really afford the .com right now, being laid off and all).
Do you guys think a website like this is necessary? Obviously the audience is mm patients that would have trouble filling out the forms themselves.
