Question about Application Paperwork

currently im a card holder in Michigan, and is going to get my wife a card, according to the law, if you are on SSI or medicaid, you qulify for a 25$ fee instead of 100...my question is...what MEDICAID documents do they need? specifcs will help, i do not wanna get rejected due to improper paperwork. thank you in advance.

OK. She mumbled that all she has to do is mail her social security letter with it. You periodically get an award letter of letter from social security stating that you get it and how much and blah blah blah. It is used for all kinds of shit. She says that is it, has been legal, been renewed all that. Hope that helps, she will be up around 2 if you have more questions. lol.

correct. mail the accepted letter from SSI (a copy of it) in with the app, and a 25.00 check.. that should do it.

If you don't have that letter from SS, you can go to your local SS office and they will print you one. The line sucks but it is FREE!!!
