Broken CO2 Reg-1 regulator - worked before tank switch - must have taken off wrong?

I have a standard Reg-1 CO2 regulator, that I only used 1 cycle. I swapped out the tank, and I dont know if I took the regulator off incorrectly - apparently so! - cus now when I go to screw the main lugnut back onto the valve thread on the tank, it threads correctly, but the stem that connects to the lug nut that runs parallel to the tank, it just doesnt seat well anymore.

Its hard to explain, but it drops down like a 1/2 centimeter, so you can clearly tell its not securely one piece anymore; the lug nut moves on and off that area a centimeter or so. very tiny, but it moves. i cant screw, push, twist, this stem any tighter into the lug nut/thread - but if so, this leak would stop.Was there 2 washers in there? Cus I only have one now. on the inside of the lug nut area.

Now when the CO2 reg is powered on, and I open the main valve, its shooting out the side.

I didnt 'effin get rough with this thing, so I dont know what I did wrong when unscrewing the main lugnut off the tank.

Any ideas?
