4 Way LST Topping Question

I'm attempting my first SCROG. I've read and reread LBH's article on the 4 Way LST and can't quite understand his instructions on topping.

I've done the initial topping and also topped the flower at the end of the 4 main branches. What about the 2 flower sites in the middle of each branch? Do they get topped too?
