The clinic will have u sign a release form and they will fax it to your Dr for your records. My drs. Receptionist called me and wanted to know if I was moving and I said no. She wanted to know why I needed my records faxed then. I said it was for a mmar, a cannabis licence. All she said was oh ok and hung up. My Dr. Never said shit when I seen him a month later.
I never asked my Dr. to sign my mmar but he may have I don't know. He is getting older and if he retires next year when I need him to sign again I'm screwed.
