What kind of paperwork do you fill out when getting a prescription?

I have my medical documents ready, but I'm curious what do they ask you when you fill out the paperwork?

my 2 questions (im in los angeles)

1) Do you need to put in your drivers license number?

2) What about Social Security Number?

Med records are private but you need to be a resident.
Why worry! Records are safe example if you work for the gov they dont turn over info.

Well the reason i ask is a lot of these doctor places look kinda shady....im a cali resident. Im just curious what info they take. Do they take ypur social security?

They make a copy of your drivers license. They don't ask for your ss #.

i had to fill out a small amount of paper work ( i mean like 1 sheet) and read a 2 sided sheet and sign/ date the bottom. like DSM said they take a photo copy of your lic, no SS card was needed. the " nurse" took my Blood pressure ( or lack there of... she didnt know how to use the damn thing. i did it for her ) 20 mins or so later i was walking out the door and headed to harborside

now this is the place i went to. my buddy went to one and saw a doctor that put him through a longer exam. plus this doctor also required a medical history. But if you are a VET you are pretty much a sure thing PTSD.... for the win

paperwork is just kind of formality...
just fill out the form and need to be attached these things :
You will need to bring a valid LA Identification Card - Valid LA I.D. means a LA Driver License or DMV issued I.D.

no need to bring SS number...

and not to worry their behavior like a sort of friend

LA state ID or drivers lic? i dont think they have passed medical pot in Louisiana.
