what forms required to change from Patient to Caregiver Mi MMJ Card

I currently have my MMJ Card but it will expire soon, and I wanted to apply for a Caregivers
Card. Is there a different form that needs to be filled out when I reapply, or just how do you switch. Thanks in advance for any information regarding this.

I would think the rules are fairly clear dude...you have to fill out the necessary form registering as a caregiver should you choose not to renew your patient registration. the form is is available on the state dhs's website. caregiver and patient application process is completely different. there is no such thing as transferring status from patient to caregiver for a registered patient. should you choose not to renew your patient application yet would still like to grow for other patients, you would have to register as a caregiver for some patient that would like you to be their caregiver.. there is no such thing as a "transfer". hope that helps...

You have to have a patient sign you up as their caregiver to be a caregiver. You do know you can be a patient and still have other patients sign you up as their caregiver dont you?

Patient card is better you have more rights and protections being a patient vs a caregiver - secondly you can't just choose to be a caregiver, a patient has to make you a caregiver - and as said above patient are allowed to grow if they don't check the box that gives possession of their plants to their caregiver. You can still have a caregiver and be the one that grows and also you can stil be a patient and also a caregiver for someone else. Also technically a caregiver who is not a patient is not even supposed to use marijuana but whos tellin ya know?

if you are a self growing pt, that is a patient with the ability to grow your own plants, your good to go.

BUT to be a CG for someone else, that person has to APPOINT you as their CG when they fill out their card, or they can file the paperwork after they have their card, naming you as their CG. you cant take patients, you Accept them, after they nominate you as their CG via the MDCH.


so have your prospective pt, file the proper paperwork with the MDCH, and they will appoint you as the CG. you will have to sign an affidavid stating you WILL NOT SUPPLY ANY Non APPOINTED PT with any MMJ once you sign the affidavid, your signed pt is the only person you can deal with, as i interperit the law. but ofcourse its this area that Dispensorys are getting thru on. SO.....

but the pt tells MDCH who their CG is, and as long as that CG has no Drug related Felonies, she/he should be ok'd to be the CG of the nominating PT.
