Looks to me like you have to high a PPM on your nutrient mix.

It's high in phosphorous like it should be, but that's such a ridiculous ratio. Just dilute it a bit more when you feed it. Also if you are giving it nutes every watering, hold off at least to every other watering, that soil will hold on to those nutes for at least a day.
