Setting my rather twisted sense of humor aside for the moment ... it looks to me like a case of porcine hydrocephaly. In humans the condition is called "water on the brain". I leave it to those with a cast-iron stomach to google pics of hydrocephalic human babies. Just ... eeewww.

Whoah that's creepy.

Poor little thing. I've raised pigs for a while and have seen a few mutations like that. The weirdest one was of a piglet that had the face of the creature from the movie "Predator", it had a split lower jaw and larger than normal tusks. They usually don't live long as they have no way of being able to eat and drink though.
