HPS bulb and ballast.

Depends on how much you want to spend and if you want to pay for a name brand or not. If brand isn't a factor and you are on a budget check your local craigslist or classifieds. I just picked up a Lowbay 400W hps with ballast, bulb and dome reflector for 30 off craigs. Nice thing about this is the ballast came in a heat dissipating baox with cooling fins. All I had to do was attach two cords which I had around my garage anyways. From what I could find the regular HPS bulbs are fine for flowering. Sure you can spend 40 on a bulb thats a little better from Hortilux or another fabulous brand but the generic off the homedePOT shelf bulbs I've seen some good results from other peoples journals on different forums. Also you didn't specify Wattage. DIY 150W will run you 75 brand new. Check out your lowes lighting section for the 150W'r. Homedepot carries some in the lower wattage ranges too.
