Help she all down hill

New plant med
50% top soil
20% perlite
25% sand
5% item 4
was watering with 15-30-15 low mis about 1/4
now just plain water
new pot.
Pic will stated with good plant and now almost dead

Whats the pH of your water that you are feeding it, and also what is the pH of the water runoff coming out the bottom.
 
you could try bringing the ph to 5.5/6....& maybe add a few drops of hydrogen peroxide ...that has worked for me when my plants were having root problems & they looked a lot like yours does
 
I had a idea that it was the root's so I put her in a bigger pot with a friend and look at the root's the only thing that got us looking was that the root's seem to be growing up not down. so we flush her, with care we got the root to hang and repoted her. I'll work on the ph problem now and i never tryed the hydrogen peroxide but i'll try to see if it work's.
 
All you need to do is flush the plant, you are clearly overfertilizing. There is no need to repot or use the hydrogen peroxide there are no signs of root rot.
 
The reason I repoted the plant was that i used mg soil that had slow release nut in it.Also i could see that the root's were growing up.I flush two ago.

Here is what she look like now, my main problem is that the new leaf are drying up.

Since your plant is still in veg stage then you need a different food with an npk ratio that looks similar to this 8-4-6. They need more nitrogen than they are getting. The food you are using is for flowering plants, this could be causing some of your issues.
 
so i changed the soil , flush almost all the old soil out"By this i mean i removed all the soil from the root as if it was hy set and planted it in the new soil.also change my nuts to 8-4-4. here are somr pic she is geting better.i add a new fan to the set-up .
