Caregiver Change Form

Hey Guys, just a few questions about the Caregiver Change Form. I have recently been notified by my Caregiver that he is no longer going to be growing, so I was forced to find a new Caregiver. I'm going to do the paperwork with him here shortly and wanted to know if I need to use the green form on the Envelope (the same one used when sending in your rec,) Do I need to include a photocopy of our drivers license? When is it actually official? When we sign the paperwork or when it's processed and we have the hard cards? Thanks for any help!

Not sure what you mean by the green forms but the forms you will want to send is the change forms (get from LARA website)..


Yes send copy of front and BACK of Drivers Licenses or ID. Make sure to fill out all info including CAREGIVER ATTESTATION and answer all applicable questions.

According to LARA you will have SECTION 4 protection with your caregiver once you receive your HARD CARD.

Not sure when it's official, I'd assume as soon
as it's signed but some people wait for the state
to cash the check (they are pretty damn quick
when it comes to cashing checks:bigjoint:). They keep
a copy of the cashed check until the new hard card
comes. The also keep copies of all paperwork mailed
to the state, proof that it was mailed, and the return
receipt then they get it back. All that crap can be
incinerated when the hard card comes.

ALWAYS send shit to the state via certified mail with return
receipt (the green forms). Hold on to the return receipt when
you get it back.

It should say on the paperwork if they want photo
id from both people or not or not. I'd make a copy
of of both and stick them in the envelope with the
paperwork. No reason to get denied over some dumbass
reason and have to mail all the shit in again.

Thanks ozzrokk and treetop. How does it work with my old card? Is it still valid or is a new number issued each time you get a new card? Sorry for all the dumb questions.

You will get a new card but the same number and expiration date. You are covered through the whole process. The new caregiver will be covered under section 4 as being connected to you once he/she receives the hard card.
