helmet head

Hey all. I'm on my first plant and she does not seem too good. She sprouted about 3 days ago. I am not sure what I have done wrong. It had the shell stuck on her until yesterday when I removed it with tweezers so could that be why it is looking like this? Or maybe I am not watering enough or too...
