When your Dr. "just says No"?

Awesome news ! As soon as He sees the Dr & gets the letter it is only a matter of a around a week at the most to recieve the card in the mail. The AZ Health Dept will O.K. you almost immediately ! I had an email waiting when I got home saying I had been accepted & the card was in the mail ! Best of Luck !!!

Thanks to all that have responded and PM'ed me. Just wanted to give a quick update (and of course, ask a couple of questions!).

We finally received the medical records this week. Tomorrow The Husband is calling to make an appointment to start the paperwork process (thanks YThor for the recommendation close to home. PLUS az420md has a $50 office visit special through the month of January advertised in The New Times 1-12-2012 edition...what's not to love about that?!?!). Okay - here are my questions regarding the application process:
  • Signed and dated State Medical Marijuana Application Form - is this something the patient provides?
  • Physician’s Certification - what is this?
  • Caregiver Attestation (if applicable) - huh? What or who determines 'applicable'?
  • Photo ID - got this one figured out :wall:

If you have a food stamp card you can cut the cost to the state to $75 and some doctors will do that too.
Your doctors logic has favored his personal believes.

What you do is call this number in Scottsdale 480-994-0420

They will help you get legal.. You will need the Full name of your doctor and their office phone number..
Next, contact the number above with the info... They will request medical history from your doctor.. They will review the medical records and contact you if approved...

Which should NOT be a problem...

Then schedule an appointment to get your medical marijuana certificate. They will help you submit all your documents to Arizona Department of Health. Then wait about 3-7 days and you will have your Medical Marijuana Program ID...

PLUS
$150 to submit paperwork and receive Az MMJ Program ID

Yeah, definitely just request his medical records from his Dr.. You don't even have to tell them why. You can always just tell them it is for your files at home. He may have to sign a medical records authorization or release.

I went through AZMMCC, and everyone was really friendly. They keep you informed through the entire process, and let you know what is needed next. You can do quite a bit online through them, and they can even send a request for records for you... But if you plan on continuing to see that Dr, it may be best for you to get the records yourself and fax them to AZMMCC or any other place.

Also, once you get your card.... You can either grow for yourself, or appoint a caregiver to grow for you.. Most caregivers will give you a set amount a month, and then of you want more you would need to reimburse them for power/ nutrients/ water/ time etc. You would just need make sure you have a agreement, written is always best. You have all the options in the world, and nobody should stop you or your husband from wanting to help him feel better. Good luck!! Help can be found anywhere, you just need to know what questions to ask to put you in the right direction..

You download the papers from the AZDHS site found on this page http://www.azdhs.gov/prop203/patients/adult.htm
The cirt you get from the dr and you scan it and email it with all the other papers.
This depends on you if you wish to grow nvr mind this cause you cant grow and have a care giver. If you are going to have a caregiver they can help with this part.
don't forget there is a Patient Attestation Form that is diffrent from caregiver and dr. Make sure you have this cause I forgot the first time .

I want everyone that has offered help and encouragement to be the first too know...The Old Guy is getting a card!!

We followed a referral from YThor and went to AZ420MD in S-dale. The office staff and doctor were great - very knowledgeable and helpful. It was $50 for the office visit and $150 for the state - $200 total. The Dr. did all the paperwork, took the picture, faxed it to the State, etc. - and it was done. He was told to expect his card sometime this week.

I feel like we just ran a half marathon! I realize that we have so much more to learn - he is approved to grow too and until then...well, we'll worry about that when the card arrives.

For today I just want to say THANK YOU, THANK YOU, THANK YOU to everyone that responded to my original post and who have PM with suggestions and support. I am trying to keep this guy around for a long time.

We look forward to getting know more like-minded folks and continuing to learn from those who have forged the way - Sissy
