What does this mean when the root showing out of the stem ?

Its growing more roots. Those are most likely anchor roots forming. Just cover them back up with a little medium, Nothing to worry about.

some roots actually need air

look to the volume of perlite in you soil

for guidance

You may be keeping your medium too wet.

So basically I’m not letting my soil dry out good enough .. and meaning that I’m overwatering cause of this a well?

Those roots are totally normal not caused by anything you are doing. I imagine you just uncovered them when you watered. From the pic though the upper leaves are definitely showing signs of nitrogen toxicity. I would back off on the N your are feeding them.
