Dr. William Ortiz has an office in Caribou, but will do house calls if the circumstances allow. I've not used him, but he posts on the CCM site every now and then. Seems like a pretty good guy who is very concerned about low income patients. You can contact him at

Unless you are planning to grow and provide meds to other patients as well, what you described is a patient certification not a caregiver card. In fact, one need not even be a patient in order to be a caregiver. What you want is a dr certification for yourself, and the state needn't have anything to do with that. If you also want to grow for others, that's a separate process.
So if it is a patient certification card any doctors in mind sorry not familiar whith the process caribou is to far but thank you for the info I appreciate the feedback guys great website
As i said before, he does house calls. He also practices in Massachusetts so, presumably, he drives down 95 fairly often. I know that he has made house calls for other patients when he is in their area. It's probably worth calling him.
