Might be moving back

Hey all in CO. I grew up in Clear Creek county til I was 12....then moved to NV. I hate it out here just stuck right now ya know! Heres my question for all you med patients in CO! What are the regulations or qualifications required to obtain a card in CO. I still have family over there and non of them have a card, just some good old outlaws trying to feel alright! I have a bulged disc in my lower back that may require surgery some day don't know for sure.... and my spine is deviated about .5" in the same region....meaning I have constant back pain, for that matter (hence the fact that I'm currently unemployed due to this condition).
A friend and myself are planning to move either back to CC county or to Summit county (heard there are logging jobs there due to the beetle kill)...seems how the med scene has exploded in CO I really want to get my card when I get there.

So heres the question(s):
What information can I get started on in the mean time before I move?

Where is the easiest and least expensive place to go about the whole process of recieving a medical card.

Also...this might just be a rumor, heard that Breckenridge de-criminalized marijuana to the point that you can walk into a dispensary without a card....I know you can get away with somethings at a couple of places in Nederland and Boulder... but that would be quite a drive if I move to Leadville! Just wondering how true this was?!
