It's called 'Motor Neuron Disease' (scene from 'The Theory of Everything')

It's incredible that a this rare disease is inflicted on a man with a one in a hundred million brain. Two years life expectancy, he certainly showed them. What a rich, full life this dude has lived. Much more full than most of us with all our faculties. Amazing...
His story is absolutely heart wrenching! The biopic about it is so well done it's tough to watch on an emotional level, this scene gives you a good taste of that.. Eddie Redmayne, the guy who plays Hawking, won the Oscar for best actor for his performance in this movie.

This scene is just too much!

The actress in that scene is Felicity Jones, who plays Jyn Erso in Rogue One (the new Star Wars movie coming out this weekend). She also gives an award winning performance throughout that movie.
