Hello everyone as a newbie of 100% perlite hempy i got these problem with this situation. Still confuse about overwatered or lacking of nutrition cause this. 
First true leaves turn yellow.

Should i add some Nutes at this stage ? Usually i put water until it reached the run off, and wait until it moist to water again.

Yes ... if the Hempy is configured right ( drain to waste ) you will NEVER OVERWATER.
You can mix up a regular veg feed and add to container.
Hempy like PH 5.5 to 6.0 ... I do 5.9. To allow for minor drift.
Nutes ... I use GH MAXIGROW and MAXIBLOOM ( powder ) at 1 tsp per gallon tap.
Nothing else.
I use GROW until 1 week AFTER flip ( plant will need the nitro to stretch and finalize structure.
At week 2 of flower ... switch to BLOOM.
I do a FEED / WATER / FEED style schedule.
The watering in between WILL clear out salt. So it is not building up.
Water is also @5.9 .
I water OR feed til it runs out a bit .... refreshes Rez , every time you allow some runoff.
