Cutting in water (dewey mister) cut stem got mushy

The bottom part of the cutting suspended in water got mushy. This never happened before, why does this happen?
 
Maybe you did not cut it well or some air bubbles got into the plants "blood" stream and caused a blockage? I had this happen a few times and i think it was because the cut was not good enough and caused the plant not to be able to drink and basically started to die where the cut was made. They stayed alive for quite a while despite dying in the end.

It did not grow any roots, right?

I just use tap water in my bubble cloner and it usually gets above 90% success for my cuttings.
I use a sharp, sterile razor when cutting them, and always do a reservoir change after the first day or two~ Any ones that start to mush or droop severely get tossed out so they don't stank up my water. The key is a clean, sterile, oxygenated environment. Also temperature of the water is important, and I don't think any rooting hormones are necessary at all.

I have 1 with roots, today is day 10 prob all cutting have roots or close to it.... I put the bottom 1 inch of the cutting in the water and only the bottom 1/8 of an inch seem mushy some.

Every time when i had that happen, it did not make roots, but the mushy part just spread and the cutting died after a while. If its about the same stuff, hard to be 100% sure without pics(or even with them), but sounds like it.
