Just a couple of ideas; I would clean up and defoliate the bottom foot or so and inside the plant to facilitate air flow, and you might find it easier to work on the bottom of the plant if you had mesh large enough to put your hands through. Good looking plant!

Is it likely i will get some extra tops by flower time if I defoliate? I've been thinking about it for a bit now but a bit timid to. Sorry if this is a dumb question.
I'm talking about defoliating at the bottom to lessen your chances of mold and mildew issues. I personally wouldn't top that plant anymore because it is pretty compact already. I would just try to get that plant to the finish line as it is, healthy. Here is what I am talking about.
