    I use peat moss with perilite. I amend with dolomite and a little EWC, some mycos. I have used coco coir with some success.

    I'd chop the tips of the leaves touching soil. The white spots look like some kind of bug damage. Could be some cal-mag deficiency.

    Why not keep it going? Are you running out of room for the other three? Chop the dead stuff off of her. Does the strain have a herm tendancy? I wouldn't worry about it.

Are you using any sort of beneficial bacteria or microhaze? You said you were growing in roots organic...

    Could be too burnt by being too close to lights. Could also be that the soil is spent and she needs some nutes. From this chart looks like Mn or Fe def.

    Perhaps oxegenating, (if that is a word) your water before you water them could bring some fresh O2 to the root zone. I bubble my water with an aquarium air pump, at least half an hour.
