Well the reason i ask is a lot of these doctor places look kinda shady....im a cali resident. Im just curious what info they take. Do they take ypur social security?

I have my medical documents ready, but I'm curious what do they ask you when you fill out the paperwork? my 2 questions (im in los angeles) 1) Do you need to put in your drivers license number? 2) What about Social Security Number? Thanks
