Looking to get my med card

Integr8 Health, Dr. Dustin Sulak in Falmouth, and Hallowell Maine 207-482-0188
www.integr8health.com. I helped my Dad get his appointment. We called Last Monday and he had his appointment yesterday. I know there use to be a three month wait but not anymore. They can get you in right away. Also Your first visit is 300.00 but if you make less than 1500.00 per month they have a sliding pay scale. I don't know what kind discount. I heard the receptionist explaining it to another patient.

I've done everything but the phone interview with Sulak. Smooth as silk in 2 weeks--would have been shorter but I had missed an e-mailed message.

Is insomnia a medical condition that qualifies? MD documented and have been on a few meds with documented failure and bad side effectes documented by two MD's within the past 4 months. Have had the condition documented for years by the two MDs.

I highly doubt it....

Me too. How about my chronic pain from artery surgery in my leg 3.5 years ago? Documented surgery yet no documented treatments for pain by an MD.

Do you really think these will qualify? Both are true and both are well documented. What Doctor would you recommend?

I used Dr. Sudlak (I think the card was actually issued by Dr. Lerner) in Falmouth. I qualified for the "sliding scale rate" ($200) and now have my certification AND my card (from the state). Just waiting for appt. w/the dispensary now.

From my friend's experience, Sulak will make you wait 6 months if there is no documentation for the pain. There is a doc in southern Maine who won't let you make an appointment unless he believes are eligible for a rec. Can't remember whether it's Phillips or Spiegel though.

I went to Integr8 Health and they get you right in. They also understand that some people avoid the doctors even if they are in pain. They also seemed to understand that some people just wanted off the pills they were on. With or without a regular Doctor, I would recommend them. They will help as much as they can.
