Golfers Elbow/Ulner Nerve Entrapment

I have golfer's elbow/ulner nerve entrapment and often wake up in the middle of the night. I am currently looking for a good golf area to move to and am choosing between AZ, NM, and NV. From my name I'm a golfer and its a condition that doesn't feel the best with what I do and keeps me up at night. Does anyone have an experience getting approved for a medical card in this condition. I know CA is an option, but its really expensive for me.
