Change of Caregiver

If i send in a change of caregiver form, do i as the patient have to use my return address or can i use my caregivers address?

I remember some issues early on with the wrong return address (caregiver or mmc rather than patient) getting the application sent back. I would just put the same as is going on the card just to be safe.
