you're missing a pretty crucial component, lime or oystershell flour. you have no medium buffer.

as for a recipe, per cubic foot (7.5 gal) of soil i would do

1cup all purpose
1/2 cup blood meal
1/2 cup potash
1/3 cup fishbone meal
1/4 cup epsom
1/2 cup oyster shell flour

use the mykos at transplant, don't put it into your soil mix.

you also need some worm castings or other good source of compost ~20% of your total volume of soil. You'll probably need to add some more drainage like pumice or perlite.

yeah lime is fine dude. same ratio. 1/2c per 7.5 gal of mix

your overall mix should be ~40% perlite, 20% compost, ~40% peat. you should be able to figure out how much of what you need to add. and then mix in the amendments as i posted above.
