    Default Quick Question!!

    So I've got my doctors appointment to get my card tomorrow. When filling out the form should I use the address on my license or the address to my apartment that I just moved into? I really don't want to get a letter of denial in the mail so your help is greatly appreciated

    I would use your new address if thats were your going to be living. You will probable change the address on your license soon anyways, that way everthing matches.

    From personal experience I can say that the address on your license does not need to match up with your current household. They just want photo verification even though they do not use the image on the med card.

    I'd use a po box so they dont have your home address.
