That is the major issue. They want records from the last 3 years that document my condition. I fiercly avoid doctors, because their methods are typically to give me some medication that doesn't help and often makes me sicker.
So I guess thats what Im asking. Do I need a recommendation from my doctor (my primary provider) to get the recommendation from THCF? Or can I just bring my medical records to THCF without a doctror's recommendation?
The way that THCF put it, it sounded like I need a doctor's (M.D. or D.O.) recommendation for MMJ. ?

Im not sure what you're asking, but if you havent already then go to THCF and get your license and then just tell your doctor you're a medical marijuana user. thats what i did. its just like telling them that you smoke cigarettes (when they ask you). they cant do anything its confidential. and if MMJ helps your migranes then why do you need a primary care provider anyways??
I mean you wouldnt need them for anything if the MMJ helps your pain!
Oh and the license is $200. make sure you bring cash!

we only had to provide 3-5 documents from within the last 3 years that showed my condition. they just want the medical record that your DR writes in when you go in. they dont want 3 years of medical records. they even state it on their webpage. just follow the directions.
you dont need a recommendation. the doctor there is the doctor that will recommend it. Step 1 If you are a NEW PATIENT, in order to schedule an appointment (FIRST YOU MUST HAVE:) * CHART NOTES (The notes your doctor keeps in your medical file.) The records need to be current within the past three years & must pertain to your qualifying condition.We only need chart notes from 2-3 recent visits on your condition. These can be UP TO three years old. For example, if you saw the doctor in September of 2006, those records could be used until September of 2009. Please do not send older documents unless requested. * These notes must be from an MD (medical doctor) or a DO (doctor of osteopathy). Oregon patients may be able to use records from a NP (nurse practioner) or a PA (physician's assistant). In other states, our doctors cannot use records from a NP or PA. * Any recent MRI or x-ray REPORTS (not required, but desirable if applicable). Please don't send the film, just the report. | | yeah i have, just wondering if i can get a MM card for anxiety attacks. i get them real bad and it makes me feel like a cant breathe and i rarely get sleep because they usually come at night. is this covered in WA. i read up on it and found a document from 2004 that says no but couldnt get anything more recent
