Hey all, I have a couple of medical users who are looking for some sort of legal form to designate a caretaker so that there is no question as to the legality of the form. If someone could help out with a link to a printable form or email it to me that would great.

What country are you in? If you are in canada go to health canada website they have everything there printable forms etc. How to Apply - Medical Use of Marihuana

Any signed dated peice of paper is legal. If you want to make it even better have it notorized.
