Once the Doctor signs your MMAR papers, and you fall under Category 1 or Category 2 conditions, HC cannot refuse the authorization. They can only refuse an authorization if the documents are incomplete. Your specialist is not contacted by any party in my experience.
Although, I'm unsure how it would work if your specialist and the signing doctor were one and the same person.

This is what I thought, but I did get the information from someone fairly close to the system. After reading and reading and reading, I can't find anything to substantiate this so I'm just going to hold my breath until that HC letter comes.
