Qualifying for a Card

Mmj is surely an option for you. There are many great mmj doctors in the valley most will see you without charging unless you qualify. I am not a doctor and would not definitively know, but it seems that your condition would qualify. What you need to do is schedule an appointment with an mmj friendly doctor and bring with you your medical records from you PCP stating the condition and its documented history. The doctor will evaluate you, review your records and then either write you a recommendation or choose another option. Then you submit the doctor recommendation along with a photo of you, a copy of a signed attestation form stating that you wont divert medicine to nonpatients, and a copy of a valid ID along with a 150$ fee to dhs (75 if you are on food stamps) and then dhs will review your application and send you out a card if you qualify.

I am going to make an appointment and see what happens. I have my medical records from my pcp and will have some from the chiropractor soon. I would give anything for a full night of pain free sleep. I have forgotten how it feels to get a good nights sleep.

You know what will happen? You will get approved. Just go to a MMJ friendly doctor they even have quite a few doctors who will give you your money back if they cant get you qualified.. You will be qualified dude, just play the part.

Check the New Times for doctor coupons. Could cut your cost in half.

no problem glad i could be of some help. and once you are approved you should hit up hydrobreed dot com. it is a forum like this except it is only for arizona and patients and caregivers are verified and you can find good caregivers with medicine there.

yep i would agree with that. he does not have to play the part, it was written for people like him.

Well I just saw my pcp again today, just prescribed more muscle relaxers and anti inflammatory meds. I'm gonna make an appointment at Phoenix Green Cross and see what happens. Thanks again for all the responses.
